Short answer
Generally no. Most Hanafi muftis say covering a scar or vitiligo patch with a tattoo is still tattooing, which the Prophet ﷺ cursed. Medical tattooing for a real need, such as radiotherapy markers or reconstruction after surgery, is a separate case for a mufti. For scars, makeup, laser and other treatments are better options.
Tattooing is haram, including to hide a scar for appearance. Where there is a recognised medical need, such as treatment markers or reconstruction after a mastectomy, some contemporary muftis permit it as repairing a defect; this depends on the case.
The four schools agree on this.
Explanation
The Prophet ﷺ cursed the woman who tattoos and the woman who has it done. The curse has no exception for beauty, and a cosmetic tattoo over a scar is still ink placed permanently under the skin to improve appearance. So most Hanafi muftis do not allow it; see why tattoos are haram.
At the same time, Islam allows repairing a real defect. When Arfajah lost his nose in battle, the Prophet ﷺ let him replace it with one of gold. Using this principle, some contemporary scholars allow medical tattooing when it serves treatment or restores a body after illness, such as small radiotherapy dots or reconstructing the areola after breast cancer surgery. These are personal cases, so ask a mufti about yours.
- Better options for scars: camouflage makeup, silicone gels, laser, steroid treatment or surgical revision, as your doctor advises.
- Vitiligo: there are medical treatments and cosmetic covers that wash off.
- Scalp micropigmentation for hair loss is tattooing; see is a hair transplant haram.
If you already have a tattoo, your wudu and prayer are valid; see is wudu valid with a tattoo. A scar is not a fault in Allah's sight, and many people carry theirs with dignity.

The evidence
Quran, Surah An-Nisaa 4:119
And I will mislead them, and I will arouse in them [sinful] desires, and I will command them so they will slit the ears of cattle, and I will command them so they will change the creation of Allah." And whoever takes Satan as an ally instead of Allah has certainly sustained a clear loss.
Satan's promise to make people change Allah's creation, the reason the scholars give for forbidding tattoos.
Sahih al-Bukhari 5937
Sahih... لعن الله الواصلة والمستوصلة والواشمة والمستوشمة ...
"Allah has cursed the woman who joins hair and the one who has it joined, and the woman who tattoos and the one who has a tattoo done."
The curse on tattooing.
Sunan Abi Dawud 4232
Graded hasan... فأمره النبي صلى الله عليه وسلم فاتخذ أنفا من ذهب ...
"The Prophet ﷺ told him, so he made a nose of gold."
Repairing a defect is allowed, which is the basis for medical exceptions.
Where the ruling is recorded
- Radd al-Muhtar (Ibn Abidin), Kitab al-Hazr wa al-Ibaha, section on looking and touching. Tattooing and the difference between beautifying and removing a defect.
- Takmilat Fath al-Mulhim (Mufti Taqi Usmani), Kitab al-Libas wa al-Zinah, on the hadith cursing those who tattoo and pluck. Changing Allah's creation is a lasting change of a normal feature for beauty; removing a defect or temporary adornment is different.
