Short answer
Al-Falaq and al-Nas. The Prophet ﷺ used to seek refuge from the jinn and the human eye with other words until these two surahs were revealed, and then he used them alone. Al-Fatihah, Ayat al-Kursi and al-Ikhlas are also used in ruqyah. Reciting 68:51 for nazar is a common practice based on the tafsir of that ayah, not on a hadith.
Reciting al-Falaq, al-Nas and other parts of the Quran to protect from or treat the evil eye is sunnah and permitted, without believing that any object or ritual has power of its own.
The four schools agree on this.
Explanation
Abu Sa'id al-Khudri reported that the Prophet ﷺ used to seek protection from the jinn and from the eye of humans until al-Mu'awwidhatayn (al-Falaq and al-Nas) were revealed. When they came, he took them and left everything else. Al-Falaq ends with refuge from the evil of an envier when he envies, which is the root of the evil eye.
- Morning and evening: al-Ikhlas, al-Falaq and al-Nas three times each, as part of the morning and evening adhkar.
- Before sleep: blow into your cupped hands, recite the three surahs and wipe over your body, three times, as the Prophet ﷺ did every night.
- For someone already affected: recite al-Fatihah, Ayat al-Kursi and the three surahs over them and blow lightly. See how to remove nazar.
Surah al-Qalam 68:51. Many commentators, Ibn Kathir among them, explain would almost make you slip with their eyes as the disbelievers wanting to strike the Prophet ﷺ with the eye, and take it as proof that the eye is real. On that basis many people recite this ayah and the next one for nazar. That is fine as ruqyah, as long as you do not believe it carries a special promise the Sunnah did not state.
The four schools agree that ruqyah with the Quran is allowed. Charms such as blue beads are no substitute: see is the evil eye real.
